Multimedia Equipment Replacement Fee
Replacement charges for lost, stolen, or damaged borrowed multimedia equipment.
Fields with asterisk are required.
Replacement or repair costs for loanable multimedia equipment. If equipment is damaged or broken, borrower is responsible for the cost to replace or return equipment to workable condition. This also includes late fee of $20 for each day overdue up to a maxium of $20 and $25.00 processing fee for each item.
